2. Making a Resavation: |

Complete all required sections of the Accommodation Reservation Form and fax it to JTB Kurashiki as soon as possible.(To ensure your booking is correctly received please be sure to use the Accommodation Reservation Form below.) |

You will be sent a “Reservation Confirmation”as well as an invoice for the total amount due, by February 13. Please bring these with you on the day. |
| (3) |
Participants from outside Japan are required to provide a credit card number to secure the reservation. The amount of the accommodation will be charged to that credit card. In the event of cancellation any refunds will be made onto your credit card following the Meeting. Accommodation confirmation and vouchers will be sent via email. Please bring these with you on the day.

Accommodation Reservation Forms must be received by January 26(Tuesday), 2010

4. Changes and Cancellations: |
|
|

All changes and cancellations must be made in writing or by fax to JTB Kurashiki. |

Cancellation charges will apply and detailed below. |
| (3) |
For cancellations, monies will be refunded via bank transfer. Refunds will be processed following the Meeting, and amounts refunded will be less cancellation charges and bank
transfer fees. Please note that due to administrative procedure refunds may take several days to be processed.
